Automated Enrollment
Trigger-Based Enrollment: As soon as a patient passes away, the system automatically enrolls the next of kin or family members into the bereavement program based on information in the hospice's database.
Personalization: Use data (relationship, preferences, etc.) to tailor communications and support to family members.
Scheduled Communications
Event-Based 5 cards are mailed to the next of kin on the important dates following their loved ones passing.
Customized Content The first month in partnership we build out the card designs and important steps for the family to take.
Tracking and Reporting
Engagement Tracking: Monitor which family members are engaging with the program (e.g., reading emails, attending events) and alert hospice staff if someone isn’t engaging for potential follow-ups.
Compliance Reports: Automatically generate reports to show compliance with regulatory requirements regarding bereavement support.
